Procedure for PSV Learner's Licence(persons holding any class of vehicle driving Licence earlier.)

Eligibility for obtaining a learner's licence:

• Age eligibility:20 years

Forms Required

• Form 1-Medical fitness certificate. Medical Fitness certificate (Form-1)is compulsory from any Medical Practitioner with photograph affixed
• Form 2-Application for learner license alongwith photograph affixed
• PSV Applicatin form
• Memorandum in triplicate

Documents Required

• Attested copy of existing Driving License in triplicate
• Driving School Certificate duly attested.
• Attested copy of school certificate with 8th pass as minimum educational Qualification
• Fee Rs. 150 /- for each class of vehicle appplied for
• If already holding any one transport class license educatonal qualification certificate is not required

Hazardous Endorsement

Forms Required

• From 1 Medical fitness cerificate. Medical Fitness Certificate(Form 1)is compulsory from any Medical Practitioner with Photograph affixed
• Certificate of IOC
• Fee of Rs. 200/- for Smart Card License.
• Fee of Rs. 100/- For endorsement.
