Procedure for Driving License Test document submission

Forms Required

• Form-4 Application for Driving License with pass port size photo graph affixed.
• Form-1 (Medical form ) with photograph affixed.

Documents Required

• Valid Learner’s license of category applied (Applicant can apply for a permanent driving test after 30 days from the date of issue of learner’s license.
• Fee Rs.200/- for Smart Card License.
• Fee for issue of Licence Rs.200/-
• Fee- Rs.300/- conducting test for each class of vehicle.
• Endorsement of additional class of vehicle Rs.300
